Tip No. 7
Ask the “what could happen if the controls do not work or they fail, expect the unexpected.”
Tip No. 8
Consider the hierarchies of controls model to remove the hazard where possible and as such reduce the risk. Elimination is the most effective way to reduce risk and PPE is the least effective.
Tip No. 9
Identify hazards and assess the overall system risk and cumulative risk. Only focusing in on your own specific task could introduce mitigations that could in turn add risks to other parties.
